Improving Outcomes and Managing Change in Complex Healthcare Environments

This opening paragraph informs the reader that the discussion to follow is Part I of a two-part case study designed to explore both the perspective of a current clinical nurse leader and the literature relative to seven central leadership issues. Part I addresses leadership in the context of the current healthcare environment along with the leader’s role in improving patient outcomes and managing change. Part II will address communication, resource management and professional engagement. The overview should include a brief description of the leader selected, his/her organizational position and overall responsibilities within the organization. The paper should be written in the third person. This objective perspective is akin to what you hear on the evening news. Thus, you wouldn’t say…I selected Mary Smith to interview for the case study because she is my Director. You would say….Mary Smith provided the nurse leader perspective for this case study. Mary is the Director of a 42-bed Medical-Surgical Unit at one of the largest acute care facilities in San Antonio.

Leadership and the Current Healthcare Environment

Summarize in a concise paragraph what the leader reported relative to the associated interview questions. (Interview questions: What characteristics do you value in a leader and why? Can you identify issues within the current healthcare environment that present particular challenges for nurse leaders? Recognizing that the scope of responsibility of staff nurses varies, how can staff nurses be effective leaders?)  Although this should be a concise summary, descriptive detail to include relevant quotes would be helpful to the reader.

After the summary paragraph, explore how the leader’s perspective aligns (or doesn’t align) with related current literature. Although your leader likely reported many characteristics they value; choose one for this paragraph. For example, if your leader reported that he/she values leaders that are clinically competent, determine whether the literature suggests that nurse leaders are more effective if they maintain clinical currency. Similarly, your leader will likely report many issues in the current healthcare environment that are challenging for leaders; choose one. [Note: Although Part I of the paper is due Week 4, you will likely complete the entire interview that addresses both Part I and Part II due later in the semester in one appointment. Be selective in your reflection. For example, your leader may report that being a good communicator is a characteristic he/she values. While that could be included here, likely you will focus on that in the Leadership and Communication section to follow in Part II.]

The final paragraph should include reflection on implications of this portion of the interview for practice. For example, getting back to the clinical currency example, you might comment on how obtaining and maintaining clinical expertise helps nurses become informal leaders. Again, this reflection would be in the third person. You are not specifically addressing implications for your practice; you are addressing implications for nursing practice.

Leadership and Patient Outcomes

As above, summarize what the leader reported relative to the associated interview questions. (Interview questions: What can nurse leaders do to promote evidence-based, patient-centered care? Have you found that information technology can improved quality and mitigate risk? Is so how? What can staff nurses do to promote evidence-based, patient-centered care?)  

As above, explore how what your leader reported aligns with (or doesn’t align with) related current literature about leadership and patient outcomes. As above, your leader will likely report a number of ways he/she believes leaders can promote evidence-based patient centered care; choose one to explore in light of current literature. Similarly, focus on one leader suggestion for staff nurses.

As above, the final paragraph in this section should include reflection on implications of this portion of the interview for nursing practice.

Leadership and Change Management

Summarize in a concise paragraph what the leader reported to you relative to the associated interview questions. (Interview questions: How do you approach instituting change?  What can staff nurses do to support organizational leaders during the change process? )

Explore how what your leader reported aligns with (or doesn’t align with) related current literature about change management. Choose one key point from each question for this exploration.  

The final paragraph should include reflection on implications of this portion of the interview for nursing practice.

Conclusion

This concluding paragraph recaps your case study with 1-2 sentences per section. No new information should be included here. The conclusion synthesizes the paper and gives the reader a clear picture of the multi-factorial roles/responsibilities of formal nurse leaders as well as how leadership principles apply to all RNs.
